Night Shift LPN/RN for Private Duty Client

About the position

We Care Home Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide specialized care for an extended care patient with trach, vent, and G-tube needs. This role involves working night shifts in the patient's home, ensuring their safety, comfort, and medical stability. Responsibilities
? Delivering specialized care for a patient with trach, vent, and G-tube
,
? Monitoring the patient's respiratory and nutritional health during your shifts
,
? Administering medications and tube feedings
,
? Performing routine assessments, interventions, and adjustments as necessary
,
? Communicating with the care team and maintaining accurate patient records
,
? Developing strong, supportive relationships with the patient and their family

Requirements
? Current Colorado LPN or RN license
,
? Experience with trach, vent, and G-tube care preferred
,
? Current BLS/CPR certification
,
? Strong clinical and communication skills
,
? Ability to work independently in a home care setting
,
? A compassionate, patient-focused approach to nursing
