Health Link Home Health and Hospice is a leading provider of comprehensive home health services throughout the San Francisco Bay Area and East Bay for over two decades. They are currently seeking a self-driven, detail-oriented Scheduling Intake Coordinator to join their Walnut Creek team, responsible for maintaining and coordinating schedules for home health services and assisting with intake processes. Responsibilities Maintain and coordinate schedules for home health services Assist with intake and process incoming referral information Ensure cases are assigned and staffed within established timeframes Communicate schedule updates to clinicians, patients, supervisors, and referral sources Manage high call volumes with professionalism and a pleasant demeanor Respond to urgent or emergency scheduling needs Provide direction to field staff as needed to ensure appropriate patient coverage Report scheduling changes and client concerns to supervisors Verify time records against schedules Follow up on scheduling issues reported by on-call staff Attend required meetings and training Maintain compliance with agency policies and procedures Perform other duties as assigned Skills Self-driven, detail-oriented Scheduling Intake Coordinator Ability to maintain and coordinate schedules for home health services Assist with intake and process incoming referral information Ensure cases are assigned and staffed within established timeframes Communicate schedule updates to clinicians, patients, supervisors, and referral sources Manage high call volumes with professionalism and a pleasant demeanor Respond to urgent or emergency scheduling needs Provide direction to field staff as needed to ensure appropriate patient coverage Report scheduling changes and client concerns to supervisors Verify time records against schedules Follow up on scheduling issues reported by on-call staff Attend required meetings and training Maintain compliance with agency policies and procedures Perform other duties as assigned Previous scheduling or intake experience Detail oriented with excellent verbal, written, and interpersonal communication skills Friendly, professional phone presence and comfortable handling high call volume Strong organizational and time-management skills Ability to prioritize and multitask in a fast-paced environment Strong computer proficiency Ability to work independently with minimal supervision Willingness to work occasional weekends and holidays as required to support patient care and operational coverage 1 year Home Health experience preferred Knowledge of medical terminology preferred Benefits Medical, dental, vision, life insurance, 401(k) with match Paid Time Off (up to 10 days) Company Overview Health Link Home Health and Hospice provides responsive in-home health care to individuals and families.
