We are looking for a Field Registered Nurse or our Arizona team. Join our team and make a positive impact on the lives of others! Position Overview The objective of this role is to assess the medical eligibility of individuals applying for the Arizona Long Term Care Services (ALTCS) program. It's a unique opportunity to help build out processes that will directly impact the lives of patients across AZ and work for the team dedicated to bringing affordable long-term care to as many people as possible. The Ideal Candidate Will Have • In-depth knowledge of ALTCS medical eligibility criteria • Experience with ALTCS PAS assessments • Understanding of Medicaid/ALTCS rules and clinical documentation Requirements Valid AZ Nursing License- LPN 2+ years of experience working with elderly population Has a patient centered apporach to patient education Direct Experience with ALTCS Eligibility (PAS) assessments Why work at FreedomCare?
