PRN RN - Phone Triage Weekends
About the position Responsibilities • Answer calls coming into the agency and respond appropriately by delegating tasks. • Provide nursing care to meet the patient's needs in accordance with the patient's plan of care. • Identify, communicate, and collaborate on patient problems based on assessment data. • Establish measurable patient goals and communicate an ongoing plan of care to the interdisciplinary team. • Recognize changes in the clinical status of the patient and report findings to the appropriate team member. • Collaborate with patients and families in developing the plan of care, adjusting communication style as necessary. Requirements • Must possess a current RN license in the state of Colorado. • Minimum of one year RN experience required, with hospice or home health experience preferred. • Hospice triage experience preferred. • Must successfully pass required background checks and drug screening. • Requires travel to multiple work locations. Nice-to-haves • Experience in home health or hospice care. • Previous triage experience. Benefits • Flexible schedule • Differential pay for shifts after 7pm
