LPN/LVN Chronic Care Management and Remote Patient Monitoring

About Us Advanced Clinic Testing partners with primary care providers to deliver comprehensive Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services. Our goal is to enhance patient outcomes while supporting physicians in meeting CMS and MIPS quality-care standards. Position Overview We are seeking a Licensed Vocational Nurse (LVN) with strong experience in CCM and RPM to join our team. The ideal candidate will not only perform direct patient follow-ups but also help organize and optimize the overall structure of the program — from documentation flow to compliance and team coordination. Responsibilities • Conduct monthly CCM and RPM patient follow-ups. • Review and ensure accuracy of documentation and credentialing. • Help improve operational workflows and standardize processes. • Communicate with providers and support staff on patient updates. • Track performance metrics (code counts, engagement rates, etc.). • Support training and oversight for other nurses or coordinators. Qualifications • Active LVN license (Texas preferred). • Minimum 2+ years of CCM and/or RPM experience. • Familiarity with CMS, MIPS, and electronic health record systems. • Strong organizational, communication, and leadership skills. • Ability to work independently in a remote environment. Compensation & Schedule • $27–$33/hr, depending on experience. • Flexible full-time schedule, Monday to Friday. • Fully remote role with team collaboration tools provided. • Opportunity for growth and performance-based incentives.
