Schedule: Monday ? Friday, 37.5 hours per week (including bus duties AM/PM)
Caseload/Details:
? Caseload: 1:1 care for a high school student with a tracheostomy.
? Responsibilities include tracheostomy care, individual health plan or plan of care, health evaluations, and assessments.
? The RN or LPN will also be responsible for riding the bus to and from school with the student.
Special Requirements: ? RN: Must have experience in tracheostomy care, health evaluations, and reporting to the district nurse supervisor or designee. ? LPN: Will be considered if they have tracheostomy experience and are familiar with obtaining doctors' orders for the student. Bill rate for LPN will be based on experience.
Key Responsibilities: ? Provide comprehensive care for a student with a tracheostomy. ? Assist with bus duties in the morning and afternoon. ? Develop and maintain an individual health plan or plan of care. ? Perform health evaluations and assessments as needed.
