We're seeking a compassionate and entrepreneurial Physical Therapist to join our team as a Remote Care Navigator, supporting users in achieving their health goals and making a meaningful impact on lives. About Limber Health Limber Health is an evidence-driven digital health platform designed to enable physicians and physical therapists to deliver a hybrid approach to musculoskeletal care. Our mobile app and web portal provide seamless digital support to complement in-clinic care, helping patients achieve better outcomes. With two clinical studies demonstrating efficacy at a top-ranked hospital, we're now partnering with leading provider groups, health plans, and employers to transform musculoskeletal care. Job Summary As a Remote Care Navigator, you'll play a vital role in supporting users with musculoskeletal pain, providing 1:1 coaching, and driving enrollment into our Limber Health MSK program. You'll work independently with limited supervision, utilizing your expertise in physical therapy and behavioral modification to guide users through their recovery journey. Key Responsibilities Provide outreach to members experiencing musculoskeletal pain and assist with enrollment into the Limber Health MSK program Assist with onboarding onto the Limber Health app, providing comprehensive support throughout the onboarding process Offer remote monitoring and coaching services to support members as they progress through their home-exercise therapy program Provide feedback, motivation, engagement, and support services to members to ensure they complete their home exercise protocol Schedule weekly check-in calls with all members enrolled in the program Organize and manage a caseload while maintaining efficiency Work evenings and weekends as required Requirements Essential Qualifications: Graduated from an accredited physical therapy program Active Physical Therapist (PT) license 2+ years of experience in musculoskeletal health Tech-savvy with a recent model laptop and smartphone Excellent communication, interpersonal, and presentation skills Strong relationship-building and listening skills Demonstrated user focus and passion for delivering exceptional user experiences Ability to multi-task, manage various types of users, and work independently with limited supervision Flexibility and agility to work cross-functionally in a fast-paced environment Preferred Qualifications: Health Coach Certification Experience in behavioral modification Passion for health and wellness What We Offer Compensation and Benefits: We offer a competitive salary and a comprehensive benefits package, including: Opportunities for professional growth and development Flexible work arrangements, including part-time and full-time options Remote work opportunities Collaborative and dynamic work environment Career Growth Opportunities: At Limber Health, we're committed to supporting the growth and development of our team members.
